If you have a problem with your health insurer that you’re unable to resolve, contact us for help.
We can help guide you through the health care system and show you how you can navigate through your health plan, but we cannot give you legal advice or make medical decisions. If you would like us to make an inquiry into your health insurance issue, please complete our inquiry form.
How to file an inquiry
Print out the
inquiry form. Fill out all the sections that apply to you. Be specific and identify what happened and with whom you have spoken to resolve the issues.
Describe events in the order in which they occurred and make sure that you attach
copies of any supporting information such as insurance handbooks, explanation of benefits, contracts, medical bills, and other correspondence. These documents and records cannot be returned to you, so please send copies rather than original documents.

Please include your telephone number. After we review your documents, one of our specialists will contact you.
If we determine that your issue is not one which is appropriate for this office, we will let you know and will forward your case to the appropriate agency for you.
Release of medical records
We may also need to call your insurer or your health care provider for additional information. To be able to do that, we need your authorization for release of medical records. You can print out a
release of medical records form, complete it and mail it to us.
