The Alarm Registration Application is now a fillable form, you can type in your information and then print it out prior to mailing it to our Administrative Office.
The Alarm Registration Application now has a statement next to the picture request that states:
Check here if an updated .jpg has been e-mailed for this application to change the current picture on file.
This is only optional if the photo on file is current.
As you will note the photo we have on file must have been taken within the past five(5) years. This check box should only be checked yes if an updated picture has been submitted for the current year especially if the applicant is renewing. Some applicants may have changed their appearance since their last application submittal such as weight loss, removed or added facial hair such as a beard or goatee, or may have gone bald or obtained hair. As stated this box should only be checked if an updated picture has been e-mailed for the current application at the time of submittal to our administrative office. If an updated picture has not been sent just leave the checkbox blank and we will use the same picture we already have on file.
ANY APPLICATIONS/FORMS/REQUIRED DOCUMENTS THAT ARE SUBMITTED TO ASL ARE REQUIRED TO HAVE THE APPLICANT'S FULL LEGAL NAME. NOT PROVIDING THE FULL LEGAL NAME CAN RESULT IN A CORRECTABLE DENIAL.
